INTEGRATED HEALTH PROMOTION

Definition of Health
Internationally, Health Promotion has been guided by the World Health Organisation (WHO) who has defined health as:  "A state of complete physical, social and mental wellbeing, and not merely the absence of disease or infirmity".

Social Model of Health
Health promotion is based on a social model of health where improvements in health and wellbeing are achieved by directing efforts towards addressing the social and environmental political, social, economic and environmental factors that effect health and produce inequities.

Health Promotion
The World Health Organisation (WHO) describes Health Promotion as:  "the process of enabling individuals and communities to increase control of the determinants of health and therefore improve their health".  Health Promotion represents a comprehensive social and political process.   It not only embraces actions directed at strengthening and skills and capacities of individuals, but also action towards changing social, environmental and economic conditions as to alleviate their impact on public and individual health.

Health Promotion in the Wimmera
The Wimmera PCP has a leadership role in strengthening the capacity of the service system to plan, implement and evaluate health promotion programs and to reorient services to be population health focused.

The Wimmera Primary Care Partnership Health Promotion network provides;
-    Mutual support for people working in health promotion.
-    Opportunities for networking, shared ideas, activities and improved knowledge.
-    Improved strategic approaches to health promotion in the Wimmera and;
-    A place to foster working relationships utilising a partnership approach.

Members of the network have a health-promoting role in their workplace and meetings occur on the second Thursday of the month from 12.30pm to 2.30pm at Grampians Community Health, 25 David Street Horsham.  We welcome new members to the group.  Click
