Long - Active Service Model.jpg

The Victorian HACC Active Service Model is a quality improvement initiative that explicitly focuses on promoting capacity building and restorative care in service delivery.  The ASM is based on the premise that many people have the potential to make gains in their wellbeing and that the HACC service system can facilitate this.  It puts the emphasis on timely interventions that prioritise capacity building or restorative care to maintain or promote a person's ability to live as independently as possible at home.

HACC Assessment Services are implementing the Active Service Model approach for assessment and care planning.  They provide broadly based, holistic assessments of client and carer need, opportunities for improving functional capacity and participation in social and community activities; and care care planning.
