The Victorian HACC Assessment Framework (DHS, 2007) sets out the program policy for assessment as a HACC-funded activity.  It details the requirements for delivering a Living at Home Assessment, which includes home-based holistic assessment of need and service specific assessments as key components.  The goal of the Framework is to support and build good practice in conducting Living at Home Assessments.  This requires designated HACC assessment services to build alliances with other key assessment providers, to ensure a coordinated and streamlined approach to assessment and care.

A Living at Home Assessment is a person-centred assessment of the person and their family or carer's needs, which leads to a care plan and individualised service responses.  The assessment and care planning process sits within the Service Coordination framework and takes an active service approach.

There are a range of documents (below) which are designed to assist assessment officers to undertake Living at Home Assessments.  The three PCP catchment areas in the Grampians Region have worked together to develop Memorandum of Understandings with other HACC Assessment Services in their catchment area - these MoUs act as guides to practice and are symbols of a coordinated approach to new initiatives.
